Register Your Food Facility Online with the FDA
On September 8, 2009, FDA launched the Reportable Food Registry (RFR or the Registry). This is an electronic portal for Industry to report when there is reasonable probability that an article of food will cause serious adverse health consequences. The RFR applies to all FDA-regulated categories of food and feed, except dietary supplements and infant formula.
Registered food facilities that manufacture, process, pack, or hold food for human or animal consumption in the United States under section 415(a) of the FD&C Act (21 U.S.C. 350d) are required to report when there is a reasonable probability that the use of, or exposure to, an article of food will cause serious adverse health consequences or death to humans or animals.
Estimating it will only take 2-3 hours to read the information and complete the registration online, the FDA encourages food facility operators to register their facilities online.
Simple information will be needed when completing the free, online application process. This includes:
- Name, physical address, phone number of the facility
- Same information for the parent company, if the facility is a subsidiary
- All trade names the facility uses
- Food product categories (21 CFR 170.3)
- A statement certifying that the information submitted is true and accurate and submitter is authorized to register the facility
- Name and contact information of the person submitting the certification statement
- Name of foreign facility's U.S. agent and the agent's contact information
- Emergency contact information
